Promoting Antimicrobial Stewardship in Nigeria

Since June 2017, the Nigeria Centre for Disease Control (NCDC) has led the coordination of Antimicrobial Resistance
(AMR) activities in Nigeria. Aside from the development of the National Action Plan (NAP) for AMR, NCDC has continued
to work closely with partners such as the relevant ministries, departments and agencies (MDAs), World Health
Organization, Food and Agriculture Organization, US Government implementing partners, and others to implement
various activities under the five thematic pillars of NAP. These pillars are:
• AMR education and awareness
• Surveillance
• Hygiene, Infection Prevention and Control (IPC)
• Biosecurity and Antimicrobial Stewardship (AMS)
• Research
The global projected increase in annual mortality due to AMR has led to the critical need to improve awareness and
bring about positive behaviour change on the appropriate use of antimicrobial therapy. In line with the AMR 2017-2021
roadmap, NCDC, coordinating with partners, has put systems in place to ensure capacity building of the antimicrobial
stewardship (AMS) Technical Working group (TWG). As part of the efforts to achieve this, the United States Agency for
International Development Medicines Technologies and Pharmaceutical services (USAID MTaPs) in Nigeria recently
supported and provided technical assistance to NCDC to organise a retreat for members of the AMS subcommittee from
24th - 25th March 2021.
The aim of the retreat was to improve the functionality of the AMS Pillar within the AMR TWG with the following specific
objectives:
1. Contribute to finalise the governance manual for the AMR TWG stipulating roles of the pillars
2. Finalise and adopt the terms of reference (TOR) for the AMS pillar
3. Identify and list work plan elements for AMS pillar activities for the next year
4. Identify priority elements for a capacity building plan for the AMS pillar
Participants were drawn from MTaPs in Nigeria, NCDC AMR TWG secretariat, and the AMS sub-committee. The
methodology was a two-day residential retreat with plenary sessions, small group facilitated workshops and a holistic
review of documents. There were also robust discussions around WHO benchmark actions for AMS and potential
MTaPS support.
At the end of the retreat, key outcomes were the development of critical documents for the AMS subgroup including an
advanced draft of the governance manual; TOR; elements of work plan; elements of capacity building plan; and
framework for periodic pillar meetings. The NCDC remains committed to working with other stakeholders in the fight
against AMR.

Lassa Fever
Key points
• There were 145 suspected cases, 30 were laboratory confirmed and six deaths were recorded from 15 LGAs in seven states
Actions
To date:
• Conducted 2021 Lassa fever high burden states preparedness/response engagement meeting
• Dissemination of reviewed case management and safe burial practices guidelines
• Risk communication and community engagement activities have been scaled up across states using television, radio, print, social media and other strategies
Planned:
• Finalise Lassa fever five-year strategic plan
Cerebrospinal Meningitis (CSM)
Key points
• There were seven suspected cases of Cerebrospinal Meningitis (CSM) reported from five LGAs in two states (Borno – 1 & Katsina – 6). None were laboratory confirmed and no death was recorded
Actions
To date:
• National CSM TWG meets weekly to review reports from states and plan appropriately
• Enhanced surveillance in all states
Planned:
• Continue harmonisation of the national line list and SORMAS data
• Continue to ensure that states reporting cases send their line lists and collect CSM samples
Yellow Fever
Key points
• There were 55 suspected cases of Yellow Fever (YF) reported from 22 LGAs in 11 states. None were laboratory confirmed and no death was recorded
Actions
To date:
• National YF multi-partner Technical Working Group (TWG) continues to coordinate activities across states.
• Daily monitoring and analysis of surveillance data across the country to guide response activities
Planned:
• Continue supporting affected states across all pillars of response
• Continue harmonisation of surveillance and laboratory data ongoing
Cholera
Key points
• There were 11 suspected cases of cholera reported from Atisbo LGA in Oyo State. None were laboratory confirmed and no death was recorded
Actions
To date
• National Cholera Multi-Sectoral Technical Working Group (TWG) is monitoring all states and supporting affected states
Planned:
• Continue follow up and monitoring of non-reporting states
• Continue harmonisation of the national line list and SORMAS data
Measles
Key points
• There were 540 suspected cases of measles reported from 74 LGAs in 17 states and FCT. Four cases were laboratory confirmed and two deaths were recorded
Actions
To date
• National Measles TWG is closely monitoring measles surveillance data and providing feedback to relevant agencies and development partners
• Ongoing weekly surveillance and laboratory data harmonisation
Planned:
• Intensify follow up with states to update and transmit line list
• Continue monthly measles surveillance data review
Monkeypox
Key points
• There was one suspected case of monkeypox reported from Ajeromi LGA in Lagos State. It was laboratory confirmed and no death was recorded

• National Monkeypox Technical Working Group (TWG) is monitoring activities in all states
Planned:
• Enhance surveillance for monkeypox in high burden states
• Continue harmonisation of the national line list and SORMAS data
Acute Flaccid Paralysis (AFP)
Key points
• There were 48 suspected cases of AFP reported from 44 LGAs in 19 states and FCT. None were laboratory confirmed and no death was recorded
National Influenza Sentinel Surveillance
Coronavirus Disease (COVID-19)
Actions
To date:
• National COVID-19 multi-partner Emergency Operations Centre (EOC) continues to coordinate response activities across states
• Ongoing assessment of infection prevention and control (IPC) capacity in the FCT
• Assessed and supported the implementation of triage stations in health facilities in Niger State
• Supported safe opening of NYSC orientation camps in 36 states and FCT
• Deployed personal protective equipment (PPE) and consumables to the laboratory network
Planned:
• Deploy additional Rapid Response Teams to support states
• Finalise Local Government Area (LGA)/State transmission categorisation
