COVID-19: Provisional Mandatory Institutional Quarantine Protocol for Returnees

The severe impacts of the ongoing coronavirus disease (COVID-19) pandemic on the global socio-economic, educational and political structures cannot be over-emphasised. One key area of concern has been the disruption to local and international trade resulting from the global ban on international air travels at the initial stage of the pandemic. Nigeria is also one of the countries that implemented this ban to prevent further importation of the virus particularly from countries categorised as high risk.

However, since the 2nd week of May 2020, the Federal Government of Nigeria (FGN) put in place measures to enable the phased evacuation of Nigerians living abroad due to increased demand to return home. NCDC has been working closely with Ports Health Service Department of the Federal Ministry of Health and other relevant Ministries, Department and Agencies (MDAs) to ensure immediate identification and monitoring of persons of interest returning via the Lagos and Abuja international airports

As part of measures to continue to open up the economy, the FGN recently announced the resumption of international flights from the 5th of September, 2020. In line with this, the Presidential Task Force on the 4th of September, released a provisional quarantine protocol for travelers arriving from outside Nigeria. Below are key excerpts which returnees must be familiar with before travelling to Nigeria:

1. Passengers must test negative by polymerase chain reaction (PCR) from country of exit 96 hours before departure and preferably within 72 hours pre-boarding.

2. All intending passengers are required to register via – http://nitp.ncdc.gov.ng and pay for a repeat (second) PCR test to be done upon arrival in Nigeria.

3. Return passengers must proceed on a 7-day self-isolation as per protocol and present at the designated sample collection sites on the 7th day of arrival.

4. Positive cases will be managed based on National Guidelines for COVID-19 treatment.

5. Passengers who test negative after 7 days will end self- isolation on the 8th day.

6. Return passengers with symptoms of COVID-19 on arrival will be placed in institutional quarantine.

We strongly urge returnees to abide by these protocols to protect themselves and their loved ones as well as reduce further risk of COVID-19 spreading in Nigeria.

Lassa Fever

Key points

• There were 33 suspected cases, one was laboratory confirmed and no death was recorded from 17 LGAs in three states.

Actions

To date:

• National Lassa fever multi-partner, multi-sectoral Technical Working Group (TWG) continues to coordinate the response activities at all levels

• Enhanced surveillance (contact tracing and active case finding) ongoing in affected states

Planned:

• Continue mobilisation of resources


Cerebrospinal Meningitis (CSM)

Key points

There were six suspected cases of Cerebrospinal Meningitis (CSM) reported from six LGAs in three states (Kaduna – 1, Katsina – 3 & Yobe – 2). There was no laboratory confirmed case and one death was recorded

Actions

To date:

• National CSM TWG meets weekly to review reports from states and plan appropriately

• Enhanced surveillance in all states

Planned:

• Continue harmonisation of the national line list and SORMAS data

• Continue to ensure that states reporting cases send their line lists and collect CSM samples


Yellow Fever

Key points

• There were 34 suspected cases of Yellow Fever (YF) reported from 25 LGAs in 10 states. None was laboratory confirmed and no death was recorded

Actions

To date:

• National multiagency YF Technical Working Group (TWG) is coordinating response activities

Planned:

• Continue harmonisation of surveillance and laboratory data ongoing


Cholera

Key points

• There were ten suspected cases of cholera reported from two LGAs in two states (Borno – 1 & Kebbi – 9). None was laboratory confirmed and no death was recorded

Actions

To date

• National Cholera Multi-Sectoral Technical Working Group (TWG) is monitoring all states and supporting affected states

Planned:

• Continue follow up and monitoring of non-reporting states

• Continue harmonisation of the national line list and SORMAS data


Measles

Key points

• There were 82 suspected cases of measles reported from 51 LGAs in 14 states. None was laboratory confirmed and no death was recorded

Actions

To date

• National Measles TWG is closely monitoring measles surveillance data and providing feedback to relevant agencies and development partners

• Weekly surveillance and laboratory data harmonisation ongoing

Planned:

• Intensify follow up with states to update and transmit line list

• Continue monthly measles surveillance data review


Monkeypox

Key points

• There was no suspected case of Monkeypox reported this week

Actions

To date

• National Monkeypox Technical Working Group (TWG) is monitoring activities in all states

Planned:

• Enhance surveillance for monkeypox in high burden states

• Continue harmonisation of the national line list and SORMAS data


Acute Flaccid Paralysis (AFP)

Key points

• There were 96 suspected cases of AFP reported from 82 LGAs in 26 states & the FCT. None was laboratory confirmed and no death was recorded


National Influenza Sentinel Surveillance

Key points

• The subtypes A seasonal H3, 2009A/H1N1 and A/not subtyped account for 0 (0.0%), 2 (9.5%) and 19 (90.5%) of the total influenza A positive sample, respectively. The subtypes B VICTORIA, B Not subtyped and B Yamagata account for 0 (0.0%), 8 (100%) and 0 (0.0%) of the total influenza B positive samples, respectively.

• The percentage influenza positive was highest in week 10 with 40%.


Coronavirus Disease (COVID-19)

Actions

To date:

• National COVID-19 multi-partner Emergency Operations Centre (EOC) continues to coordinate response activities across states

• Held a virtual strategic coordination meeting aimed at intensifying response with Technical Advisors supporting the implementation of the NCDC/World Bank prioritised COVID-19 response activities in 29 States

• Completed deployment of Surveillance and Outbreak Response Management Analysis System (SORMAS) tool in Osun and Yobe States

• Obtained ethical approvals to conduct clinical management study to determine the management and outcome of COVID-19 cases from the State Ministry of Health (SMoH) and University College Hospital (UCH) Ibadan, Ethical Review Committees (ERCs) in Oyo State

Planned:

• Distribution of August/September medical supplies to states and treatment centres, Federal Health Institutions (FHIs) and Primary Healthcare Centres (PHCs)

• Train staff of correctional facilities on COVID-19 prevention from 2nd – 4th September 2020

• Finalise the draft guidelines on intensified surveillance in 'silent LGAs' across the country (LGAs that have not reported a single suspected case)

• Research Pillar to investigate cases of re-infection with COVID-19 in Nigeria
