In delivering on its mandate to lead the preparedness, detection and response to infectious diseases outbreak, the Nigeria Centre for Disease Control and prevention (NCDC) intends to commission an insurance Broker for its General Assets. In doing so, the services of the insurance broker will be required to provide necessary technical facilitation.
2. in the above regards, and in compliance with the Public Procurement Act 2007, the Centre wishes to call for proposals from the general Public, with necessary documents on the proposed job as captured below:

3. The submission should be accompanied by the following documents:
i.Certificate of Incorporation/Registration with Corporate Affairs Commission (CAC);
ii.Evidence of Certification from PENCOM in accordance with the Pension Reform Act 2004 including evidence of remittances of Employees’ Pension Contributions/Deductions;
iii.Evidence of Tax Clearance Certificates for the last three years (2022,2023 & 2024) corresponding with declared turnover;
iv.Evidence of VAT Registration with TIN No. and past remittances for the last 3 years;
v.Evidence of payment of training contributions to Industrial Training Fund (ITF) in compliance with the provisions of ITF Amendment Act 2011;
vi.Evidence of Registration on the National Database of Contractors, Consultants and Service Providers (Submit Interim Registration Report from Bureau of Public Procurement).
4. OTHER REQUIREMENTS:
i. Company Audited Account for the last 3 years (2012, 2013 & 2014) by qualified Auditor and showing annual turnover;
ii. Company Profile and Technical qualifications of key personnel with evidence of experience of similar jobs; Evidence of Registration with Relevant Professional Bodies
iii. Verifiable List of similar jobs successfully executed in the last 3 years including Letters of Award of Contracts; Project Costs, Job Completion Certificates and Payments (please provide the contact addresses of the Clients) for verification.
iv. Evidence of Financial capability to execute the Projects (Bank Guarantees from reputable banks will form part of the evaluation criteria);
v. A sworn affidavit confirming that all information presented and attached are true and correct of the company and that no officer of the NCDC is a former or present Director, Shareholder or has any pecuniary interest in the bidder;
5. SUBMISSION REQUIREMENTS:
i. Completed Technical and Financial Proposals (One soft copy and two sets of hard copies) shall be submitted in two different sealed tamper-proof envelopes and labeled “Technical Proposal” and “Financial Proposal” and both placed in a third envelope. All the 2 nos. envelopes should bear the “Project Name” at the Top Right-Hand Corner of the Envelope and addressed to:

ii. The company’s name should be written at the requested side of the envelop submitted at the Office of the Head (Procurement), Procurement Unit, Ground floor, NCDC Headquarters, Jabi, Abuja.

Opening of the received documents will commence immediately after the closing on 19th JULY, 2024, at 12noon prompt at:

7. PLEASE NOTE THAT:
i. All CAC, VAT, PENCOM, TAX, ITF, Certificates may be referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue Services (FIRS), National Pension Commission and other relevant bodies for verification.
ii. The Evaluation of the received documents shall be strictly based on the requirements as specified above and as contained in the Bid Documents.
iii. The Nigeria Centre for Disease Control and prevention reserves the right to reject any or all the submissions that do not meet up with the requirements as specified.
